BIRADS System: Complete Guide

IMPORTANT NOTICE: This content is informational and does not replace individualized medical evaluation. Always consult a health care professional.

The BIRADS system (Breast Imaging Reporting and Data System) is a standardized tool developed by the American College of Radiology to classify and report findings on breast imaging exams, promoting clear communication between radiologists and treating physicians.

What is the BIRADS System?

BIRADS is a classification system that categorizes breast lesions according to their probability of malignancy, providing standardized clinical management recommendations. It was developed to reduce variability in exam interpretation and improve medical communication.

Main Objectives

  • Standardize terminology in breast imaging reports
  • Reduce confusion in communication between physicians of different disciplines
  • Facilitate outcome monitoring and medical audit
  • Support evidence-based clinical decision-making

Application by Modality

Mammography

The first modality in which BIRADS was implemented. It uses specific criteria such as shape, margins, density, and associated features to classify lesions.

Ultrasonography

Adaptation of BIRADS for breast ultrasound, considering characteristics such as echogenicity, shape, orientation, margins, and posterior acoustic transmission.

Magnetic Resonance Imaging

Most recent version, focused on morphologic and dynamic enhancement criteria specific to MRI, including contrast kinetics analysis.

Category Details

CategoryDetailed DescriptionRisk (%)Recommended Management
BIRADS 0Incomplete assessment. Additional imaging and/or prior images are needed.VariableComplete the work-up with additional exams
BIRADS 1Negative. No significant findings to report.0%Routine screening according to age
BIRADS 2Definitely benign findings. No follow-up needed.0%Routine screening according to age
BIRADS 3Probably benign finding. Short-interval follow-up recommended.≤ 2%6-month follow-up, then annually until 2-3 years are completed
BIRADS 4Suspicious abnormality. Biopsy should be considered.2-95%Percutaneous biopsy (subdivided into 4A, 4B, 4C)
BIRADS 5Finding highly suggestive of malignancy.≥ 95%Percutaneous biopsy with surgical planning
BIRADS 6Known malignancy, proven by biopsy.100%Appropriate oncologic treatment

Limitations and Considerations

  • Interobserver variability even with standardized criteria
  • Need for adequate training for consistent application
  • Population differences may affect predictive values
  • Importance of correlation with clinical history and physical examination

Audit and Quality Control

The BIRADS system allows systematic auditing of results through the calculation of indicators such as:

  • Cancer detection rate by category
  • Positive predictive value of recommended biopsies
  • Recall rate for diagnostic work-up
  • Agreement between different radiologists
