HBPM protocol (Home Blood Pressure Monitoring) — gold standard recommended by the Brazilian Society of Hypertension (2020) and ESH Guidelines 2023.
💡
Why measure at home? Blood pressure measured in the office can be up to 10–15 mmHg higher than at home (white coat hypertension). Home blood pressure monitoring (HBPM) eliminates this bias and allows your doctor to adjust treatment far more precisely. Studies show that patients who do HBPM have better long-term blood pressure control.
🕐 Ideal Measurement Times 📚 SBH 2020
Measure 2 times in the morning e 2 times in the evening, for 5 days (discard day 1).
🌅
6h – 9h
Morning — 1st measurement
Before taking any medication and before breakfast
☕
Morning + 2 min
Morning — 2nd measurement
Wait 2 minutes after the 1st reading and repeat
🌆
18h – 21h
Evening — 1st measurement
Before dinner, after 5 min of seated rest
🌙
Evening + 2 min
Evening — 2nd measurement
Wait 2 minutes after the 1st reading and repeat
📅
Duration: 5 consecutive days (discard the day 1 measurements). Usable total: 32 measurements. Your doctor will calculate the average to assess your blood pressure control.
✅ Correct Technique — Step by Step
1
Prepare the environment — Sit in a chair with back support, feet flat on the floor, in a quiet place without noise. Rest for 5 minutes before starting.
2
Avoid stimulants — For the 30 minutes beforehand: no coffee, no smoking, no exercise, and no talking or phone use during the measurement.
3
Position your arm — Arm resting on the table at heart level (sternum level). Palm facing up, elbow slightly bent.
4
Put on the cuff — 2 to 3 cm above the elbow crease. It should not be too tight or too loose (you should be able to fit 2 fingers underneath).
5
During the measurement — Stay still, without crossing your legs, talking, or moving. Breathe normally.
6
Record immediately — Write down both numbers (e.g., 130/85) and the heart rate. Repeat after 2 minutes and write it down again.
⚡ What to Avoid
☕
Coffee and stimulants Avoid for 30 min beforehand. Caffeine temporarily raises BP by 5–10 mmHg.
🚬
Cigarettes Avoid for at least 30 min beforehand. Nicotine causes immediate vasoconstriction.
🏋️
Physical exercise Avoid for 60 min beforehand. The post-exercise antihypertensive effect can mask true values.
🍽️
Heavy meal Wait at least 30 min after eating before measuring.
💊
Medication before measuring Take your medication after measuring in the morning to record the trough effect.
💬
Talking during the measurement Talking raises BP by 6–7 mmHg. Stay silent throughout the measurement.

When to seek immediate care: BP ≥ 180/120 mmHg with symptoms such as severe headache, blurred vision, chest pain, or shortness of breath — go to urgent care or call 192 (SAMU). If you have no symptoms but BP ≥ 180/120, contact your doctor the same day.

💡
Why monitor at home? Self-monitoring of blood glucose (SMBG) lets you identify hypoglycemia and hyperglycemia, assess the impact of diet and exercise, and adjust treatment precisely. Studies show an average 0.5% reduction in HbA1c in patients who monitor regularly.

Monitoring profiles (according to your treatment):
• Diet/metformin: fasting + 2h after main meals (3–4x/day)
• Sulfonylurea/glinide: fasting + pre/post meals (4–6x/day)
• Basal insulin: daily fasting + post-meal when indicated
• Intensive insulin: pre- and post-all meals + at bedtime (6–8x/day)
✅ Correct Technique — Step by Step
1
Wash your hands with soap and water and dry well. Do not use alcohol-based hand sanitizer (it can falsify the result). If you have no water, use the 2nd drop of blood and discard the 1st.
2
Warm your finger — Rub gently or place under warm water for a few seconds to improve blood flow.
3
Insert the lancet — Always use a new lancet. Prick the side of the fingertip (less painful than the center). Rotate fingers to avoid calluses.
4
Apply the drop — Touch the drop of blood to the strip (do not squeeze the finger hard — it can dilute the blood with tissue fluid).
5
Wait for the result — The value appears on the display in 5–15 seconds. Record it immediately with the time and situation (fasting, pre- or post-meal).
6
Dispose of safely — Lancets and strips must be discarded in a rigid container (closed PET bottle) and taken to a collection point. Never throw them in an open trash bin.
⚡ Common Mistakes and How to Avoid Them
🍊
Hands with food residue Sugar on the skin falsely raises the result. Always wash your hands first.
🧴
Hand sanitizer or lotion Interferes with the reading. Use only soap and water.
🥶
Cold hands Poor circulation causes inconsistent readings. Warm your finger before pricking.
💧
Squeezing the finger hard It dilutes the blood and falsifies the result. The drop should come out on its own.
📅
Expired test strips or strips exposed to heat Check the expiration date and store away from light and moisture.
🔁
Reusing lancets They get dull and increase pain and infection risk. Always use a new one.

🚨 How to Act in Hypoglycemia (Blood Glucose < 70 mg/dL)
1
Confirm with the glucometer — If you feel symptoms (tremor, sweating, palpitations, weakness), measure immediately.
2
The 15 Rule — Consume 15 g of fast-acting carbohydrate: 1 glass of orange juice (150 mL) or 3 packets of honey or 4 glucose tablets or 1 tablespoon of sugar dissolved in water.
3
Wait 15 minutes and recheck your blood glucose. If it is still below 70 mg/dL, repeat the treatment.
4
After normalizing — Have a snack with complex carbohydrate and protein (whole-grain bread + cheese) to prevent another drop.
🚨
Severe hypoglycemia (< 54 mg/dL) or loss of consciousness: Do NOT give anything by mouth. Call emergency services immediately (SAMU, 192, in Brazil) or go to an urgent care facility. If glucagon is available, administer it as directed by your doctor.
